PLEASE READ AND AGREE TO THE FOLLOWING STATEMENT: Even though FCA and Green Pastures, LLC are doing all the deep cleaning and social distancing requirements necessary to comply with the current COVID-19 prevention regulations, I understand the risk my child has to possible COVID-19 exposure through participation in the Boxing Huddle. I further release the Fellowship of Christian Athletes (FCA), Green Pastures, LLC, and both said entities’ leaders, employees, Officers, Directors, volunteers, and agents from any claim that my student may have or that I may have against them as a result of COVID-19 exposure incurred during the course of participation in the activities. This release of liability shall include (without limitation) any claims of negligence or breach of warranty. This release of liability is also intended to cover all claims that members of the student's or my family or estate, heirs, representatives, or assigns may have against these organizations or their leaders, employees, volunteers, or agents. I further agree to indemnify and hold harmless FCA, Green Pastures, LLC, and both said entities’ leaders, employees, Officers, Directors, volunteers, or agents from any and all claims arising from my minor’s participation in its activities and programs as a result of COVID-19 exposure during such activities.
